Health outcomes are determined by a variety of factors that live outside the healthcare setting, such as housing, access to healthy food, education and employment. Over the last two years, there has been a shift away from the traditional view of addressing health, which previously looked exclusively at a person from a clinical perspective towards a view that takes into account the whole person. Despite the challenges associated with transforming the health system, there are many innovative programs that are working towards building a better health system and an evolution of thinking, commonly referred to as Population Health.
